Virtual Consultations – What you need to know Patient Information

With self isolation being a huge obstacle in maintaining our health care needs, Apple Creek is pleased to offer virtual consultation appointments. This March Madness Option is a great way to continue to progress your rehabilitation while maintaining contact with your therapist during these trying times.

* To book an appointment, best would be to email apple@applecreeksports.com and indicate who you would like to consult with and which morning or afternoon is good for you. We will then coordinate with your practitioner and confirm a time and send connecting instructions with that confirmation.
* Fees: During these uncertain times we continue to make an effort to provide optimal customer service and assist you in achieving your goals. The March Madness Option will be billed as an appointment with your practitioner at a reduction of 50% to the usual cost.
* Your invoice will be emailed to you and payment can be processed by either calling in your credit card info or e-transferring the payment to paymentsapplecreek@gmail.com .
Apple Creek continues to strive to provide optimal care for the clients and team members and are taking all suggested precautions as directed by Health Canada and the World Health Organization. All decisions being made are based on the most recent updates provided to us and keeping everyone’s best interests at heart.
Stay safe, be calm and help each other. We are in this together.
